Requesting a home visit

Home visits are made to patients who are too ill to attend the surgeries.

• To enable the Practice to triage calls, please make requests before 10.30am. 
• If telephoning the surgery, please select the home visit line.
• The receptionist will take details of your name, address, telephone number, and an indication of the symptoms via our triage form, as this helps us to triage and assess the priority of calls.
• The request shall be reviewed by the on-the-day triage team, who shall decide the appropriate actions.
• This may be an on-the-day appointment or a routine appointment.
• A doctor, urgent care practitioner, paramedic practitioner, or advanced nurse practitioner may then either telephone, arrange a video call, or visit if appropriate
